Compassionate, Comprehensive Care

Ascend Rehabilitation provides traditional inpatient physical therapy, occupational therapy and speech therapy with programming specialized for the senior population. As part of our comprehensive continuum of care, we offer services for transitional care after surgery or hospitalization, skilled-nursing at long-term care facilities and short-term rehabilitation settings. RESIDENTIAL CARE

In long-term care or a skilled nursing facility, we can address client circumstances such as:

  • Difficulty with transfers, moving place to place and requiring more assistance
  • Decline in flexibility and/or pain with movement
  • Appearance of being uncomfortable in a sitting position
  • Increased difficulty communicating with staff and others
  • Coughing while dining

Ascend Rehab also coordinates with interdisciplinary teams, nursing staff, physicians and social workers to assure long-term clients receive the services needed to maintain the highest practical level of function.

TRANSITIONAL CARE

Our short-stay rehab clients receive the intensity of therapy needed during an appropriate length of stay for their condition. Prior to discharge, we offer home safety assessments and provide a comprehensive report of recommendations for equipment and services to assure a safe discharge plan. After a hospitalization for surgery, illness or injury, older adults may need additional time to regain functionality before returning home. Ascend Rehabilitation offers inpatient, transitional care in an environment dedicated to accelerating recovery. Our specialized, individualized therapy and rehabilitation programs are designed to return our clients safely and quickly to their daily activities and routines at home. 

Ascend Rehabilitation transitional care services include:

  • Specialty care for patients recovering from knee, hip or shoulder surgery, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and other conditions
  • Ascend Rehab TCU services available seven days a week
